I'll take your word for it on this occasion. When you point the "camera" at the ground it has trouble finding the object which you are trying to photograph, and the ground may be in focus but not the object. Try getting down to the level of the object, and have plenty of open space behind the object, and you should get a better photo.
If you just can't get a focus on something small like that, putting your hand just behind the grass head etc can make it easier for the camera to focus (focusing on your hand). Once focus is locked you probably can remove your hand, and perhaps moving the phone back slightly so that it is better focused on the grass rather than just behind it
Ryu, I often do that but iPhone 4S is not a good camera for closeups, it was a very sunny day and so it was very difficult to actually the image, and I knew what the grass was. Thanks GeoffR
